How to Fill Out and Submit a Form
Overview
Geo-CX includes a built-in form system for submitting safety reports, audits, incident reports, project impact reports, expense requests, and more. Forms are filled out section by section directly from the mobile app and submitted to your project team.
Some forms may also appear automatically when you punch in, if an audit is required for today's shift.
Access to forms requires specific permissions. If you do not see these sections in your sidebar, contact your supervisor or administrator to verify your access.

How to Access Forms
There are three ways to open a form:

| Automatic at punch in | If a safety audit is required for today, the form opens automatically after you punch in |
Forms shown at punch in are required. If an audit form appears after you punch in, you must complete and submit it before continuing.
Finding and Opening a Form
From the Safety Forms or Project Forms list, each card shows the form name, type, and the date it was created. Tap a card to open it.

Filling Out a Form
Forms are divided into sections. You work through one section at a time — complete the fields on the current section, then tap Next to advance. The last section has a Submit button instead of Next.
Tap the back button at any time to return to the previous section. Your answers are preserved.
Field Types You May Encounter
Different forms use different field types. Here is what each one looks like and how to fill it in:

| Text area | Tap the field and type a longer response — up to 500 characters |
| Number | Tap the field and enter a numeric value |
| Date | Tap to open a date picker and select a date |
| Time | Tap to open a time picker and select a time (24-hour format) |
| Radio button | Tap one option to select it — only one choice allowed |
| Checkbox | Tap one or more options — multiple selections allowed |
| Dropdown | Tap to open the list and select one option |
| Resource selector | Tap to open a list of project members — select one or more |
| Photo (camera) | Tap to open your camera and capture a photo |
| File upload | Tap to pick a file from your device |
| Signature | Draw your signature on the canvas with your finger |
Required fields are marked. If you tap Next or Submit without completing a required field, the app highlights the missing field and prevents you from advancing.

Taking Photos in a Form
If a form has a camera field:
Tap the camera field
Your device camera opens. Take the photo — it appears as a thumbnail in the form.
- You can add up to 10 photos per camera field.
- Tap the X on any thumbnail to remove it.
- The add button disappears once you reach the maximum.
Photos are attached
The photos are uploaded when you submit the form. You do not need to do anything extra.
Uploading Files in a Form
If a form has a file upload field:
- Tap the field to open your device file picker.
- Select one or more files — up to 10 files per field, maximum 10 MB each.
- Supported formats include PDF, DOC, DOCX, XLS, XLSX, PNG, JPG, and more.
- Tap the X on any file to remove it before submitting.

Signing a Form
If a form requires a signature:
Draw your signature
Use your finger to draw your signature on the canvas area. Take your time — the canvas does not time out.
- Tap Reset to clear the canvas and start over.
Tap Save
Tap Save to confirm your signature. It is saved as an image and attached to the form automatically.
- You must tap Save — drawing alone does not count as a completed signature field.

Pass / Fail on Audit Forms
Some forms — such as safety audits — include a Pass / Fail selector on the final section. Select the appropriate outcome before submitting.
Submitting a Fail result sends a notification. If you select Fail and submit, the app automatically notifies your supervisor and manager. Make sure the result accurately reflects the site conditions.
Submitting the Form
Once you have completed all sections and all required fields are filled in, tap Submit on the last section. A confirmation appears when the form is submitted successfully.
Forms work offline. If you have no internet connection when you submit, the form is saved locally on your device and uploaded automatically when your connection is restored. See → Offline Mode and Offline Selfie for more on how offline sync works.
Things to Know
You cannot edit a form after submitting. Once submitted, the form is locked. If a correction is needed, contact your supervisor.
Camera access is required for photo fields. The first time a form asks for a photo, the app will request camera permission. If you previously denied camera access, go to your device settings to re-enable it for Geo-CX.
Forms are project-scoped. The forms available to you depend on your active project. If you switch projects, the form list updates to show that project's forms.
