How to Report an Injury


Overview

If you are injured on the job, you can report it directly from the Geo-CX app. There are two ways to submit an injury report:

  • During punch out — if you were injured during your shift, report it as part of the punch-out flow. The report is automatically linked to that shift's timesheet entry.
  • From the Report Injury section — submit a standalone report at any time, independently of your punch activity.

Both methods collect the same information. The difference is when you use them and how the report is linked to your work records.


Option 1: Reporting an Injury During Punch Out

When you punch out, the app asks whether you were injured during your shift. This is the recommended way to report an injury that happened while you were working, as it links the report directly to your timesheet entry for that day.

Tap Punch Out

From the Shift screen, tap the Punch Out button.

Answer the injury question

The app asks: "Did you experience any injuries?"

  • Tap No to complete the punch out without a report.
  • Tap Yes to open the injury report form.

Fill in the injury details

Describe what happened in the text field and attach at least one photo.

  • Description is required — up to 400 characters.
  • At least one photo is required — upload from your gallery or take a photo with your camera.
  • Maximum 5 photos total across both sources.

Tap Submit

The injury report is submitted and the punch out completes. The report is linked to your timesheet entry for that shift.

Option 2: Standalone Injury Report

Use this option to report an injury at any time — before or after your shift, or when the injury was not captured during punch out.

You can access the standalone report from two places:

  • Sidebar → Report Injury
  • Dashboard → Quick Actions → Report Injury

Open Report Injury

From the sidebar or the Dashboard Quick Actions, tap Report Injury. The injury report form opens.

  • Make sure you have a project selected before opening the form. If no project is active, the form cannot be submitted.

Fill in the injury details

Describe what happened in the text field and attach at least one photo.

  • Description is required — up to 400 characters.
  • At least one photo is required — upload from your gallery or take a photo with your camera.
  • Maximum 5 photos total across both sources.

Tap Submit

The report is submitted to your active project. A confirmation appears when the upload is successful.

Adding Photos to an Injury Report

Both the punch-out and standalone flows use the same photo attachment method:

Photos appear as thumbnails in the form. Tap the X on any thumbnail to remove it before submitting. The add button disappears once you reach the 5-photo maximum.

Accepted formats: JPG, PNG. Maximum 10 MB per photo.

Camera access is required to take photos. If you previously denied camera access to Geo-CX, go to your device settings to re-enable it.


Things to Know

Reports submitted during punch out are linked to your timesheet. This creates a direct record connecting the injury to the specific shift it occurred during. Standalone reports are linked to your project but not to a specific timesheet entry.

Injury reports work offline. If you have no internet connection when you submit, the report and photos are saved locally and uploaded automatically when your connection is restored.

You cannot edit a report after submitting. Once an injury report is submitted it cannot be changed from the app. If a correction is needed, contact your supervisor.

Be specific in your description. Your supervisor and project manager will use the description and photos to follow up. Include what happened, where it happened, and any relevant context.
